FAB Camp will be held at Guyandotte Baptist Association Camp Ona in Ona, WV. The cost is $50 per student. If multiple members of your family are participating, please complete one form per family member.

Required items: 2017 FAB Medical Form, Image Release on file. These can be found on the church website fifthavenuebaptist.org under "Missions" then "Downloads". The cost of camp is $50 per student. Please complete all forms and submit your student fee to the Drop Box near the FAB Lab by July 16, 2017. Thank you!

Students MUST bring sleeping bags or bedding, pillows, Bible, clothes, toiletries, etc. Cash if want to purchase snack items from Camp Store (optional - there will be plenty of snacks).
