Fifth Avenue Baptist Church / Forms 3 / Youth Kings Island Trip

Youth will be celebrating the beginning of summer with a fun filled day of rides at Kings Island Amusement Park. Transportation will be provided. Youth and adults who would like to chaperone need to register. Must have 2017 medical form on file with church office. Remember if you are bringing a friend have them register and have medical form completed.
