PAYMENT for ACTIVITY:

ALL reservations will have a refundable deposit fee. To see these, be sure to look at the Personal Activity Request Form on the website under Resources / Facility Reservations. Payment can be made in person or from our website. DO NOT make payment until your activity has been approved.

CANCELLATION Fee: $100 if cancelled less than one week of planned event

By signing below you state:

I understand that set-up and clean-up are my responsibilty and that all furniture should be returned to the original set-up. I have read and agree to FBC Guideline & Policies and Clean-up Check List and I understand that failure to adhere could result in loss of deposit.
