Dear Parents,
I hope that the youth ministry site will keep you informed of the events happening with our youth. You can also check us out on iconnect or on the Youth Group Facebook Page. Below, you will find the documents you need that will enable your student to attend youth events with ACC youth ministry.

All students need a Medical Power of Attorney/Permission Form on file at the church in order to attend any event that requires transporting students in an ACC vehicle. Please note that this document needs to be notarized.
If you are attending an event with a friend from ACC you need this form: Medical Power of Attorney/Permission Form. If your child takes medication please fill out a medication form.
